A crucial distinction: detecting a chemical or metabolite in blood or urine establishes exposure, not toxicity or causation of symptoms. CDC explicitly cautions against equating biomonitoring detection with disease. European biomonitoring likewise demonstrates widespread exposure to several chemical classes.

Several of the chemicals toward the top of this table also appear prominently on the current ATSDR Substance Priority List. Its 2025 rankings begin with lead, arsenic, cadmium, vinyl chloride, TCE, nickel, benzo[a]pyrene and benzene. Importantly, ATSDR emphasizes that this is not a ranking of the intrinsically “most toxic” chemicals; it incorporates occurrence, toxicity and potential exposure at hazardous-waste sites.

Evidence that exposure is widespread

CDC’s National Exposure Report uses blood and urine from NHANES participants to provide nationally representative estimates of U.S. exposure. Its major categories include metals, PFAS, consumer-product chemicals, pesticides, phthalates, PAHs, tobacco-related chemicals and VOCs.

Europe shows a similar problem. HBM4EU biomonitoring found BPA in 92% of adults sampled across participating European countries, with BPS and BPF detected in 67% and 62%, respectively. European monitoring also identified concerning combined exposure to mixtures of phthalates among children and adolescents.

5. European Environment Agency. Chemicals and Health. In: Zero Pollution Monitoring Assessment. Copenhagen: EEA. This review incorporates European human-biomonitoring findings for bisphenols, phthalates and other important chemical exposures.

6. European Environment Agency. Human Exposure to Bisphenol A in Europe. Copenhagen: EEA. The report summarizes harmonized HBM4EU biomonitoring involving 2,756 adults across 11 European countries and measurements of BPA, BPS and BPF.

An important clinical point

A better approach is to rank them by (1) prevalence of exposure, (2) persistence/bioaccumulation, (3) evidence for human toxicity, (4) neurological effects, (5) endocrine effects, (6) immune/autoimmune associations, (7) carcinogenicity, and (8) availability of clinically validated testing.
